2. Identification of the material that was removed
3. A statement under penalty of perjury that you have a good faith belief the material was removed by mistake
4. Your name, address, phone number, and email
5. Consent to jurisdiction of federal court in your district

Repeat Infringer Policy

Users who repeatedly infringe copyrights will have their accounts terminated:

• First Offense: Warning and content removal
• Second Offense: Temporary account suspension
• Third Offense: Permanent account termination
